The liver is the primary organ responsible for metabolizing alcohol, and it bears the brunt of alcohol's toxic effects. Alcohol-related liver disease (ARLD) is the most common cause of liver disease in the United States and the third leading cause of liver transplantation. It encompasses a spectrum of conditions — from reversible fatty liver to potentially fatal cirrhosis — and is one of the most compelling medical reasons to achieve and maintain sobriety. Alcoholic fatty liver (steatosis): The earliest stage of ARLD, characterized by accumulation of fat in liver cells. Reversible with abstinence.
Alcoholic hepatitis: Inflammation of the liver caused by alcohol, ranging from mild to severe. Severe alcoholic hepatitis carries a 30-day mortality rate of 30-50%.
Cirrhosis: Irreversible scarring of the liver that impairs its function. The end stage of ARLD. Associated with multiple serious complications.
Portal hypertension: Elevated pressure in the portal vein system, a complication of cirrhosis that causes varices, ascites, and other complications.
Hepatic encephalopathy: Brain dysfunction caused by liver failure, characterized by confusion, altered consciousness, and coma. A serious complication of cirrhosis.
Stage 1: Alcoholic fatty liver (steatosis): The earliest and most common stage of ARLD, occurring in approximately 90% of heavy drinkers. Fat accumulates in liver cells because alcohol metabolism disrupts normal fat metabolism. Fatty liver is usually asymptomatic and is detected incidentally on imaging or blood tests. It is completely reversible with abstinence — the liver returns to normal within weeks of stopping drinking.
Stage 2: Alcoholic hepatitis: Inflammation of the liver that occurs in approximately 10-35% of heavy drinkers. Ranges from mild (asymptomatic, detected on blood tests) to severe (jaundice, fever, abdominal pain, coagulopathy). Severe alcoholic hepatitis is a medical emergency with a 30-day mortality rate of 30-50%. Treatment includes corticosteroids (prednisolone) for severe cases, nutritional support, and abstinence. Mild to moderate alcoholic hepatitis is reversible with abstinence.
Stage 3: Fibrosis: Repeated episodes of alcoholic hepatitis cause progressive scarring (fibrosis) of the liver. Early fibrosis is potentially reversible with abstinence; advanced fibrosis may be partially reversible.
Stage 4: Cirrhosis: The end stage of ARLD, characterized by irreversible scarring that replaces normal liver tissue. Cirrhosis impairs the liver's ability to perform its essential functions — detoxification, protein synthesis, and bile production. Approximately 10-20% of heavy drinkers develop cirrhosis. ARLD is diagnosed through a combination of history, physical examination, blood tests, and imaging:
Blood tests: Liver function tests (AST, ALT, GGT, bilirubin, albumin, INR) assess liver inflammation and function. In ARLD, AST is typically elevated more than ALT (AST:ALT ratio >2:1), and GGT is often markedly elevated.
Imaging: Ultrasound, CT, and MRI can detect fatty liver, cirrhosis, and complications. Fibroscan (transient elastography) measures liver stiffness, providing a non-invasive assessment of fibrosis.
Liver biopsy: The gold standard for diagnosing and staging ARLD, though it is invasive and carries risks. Used when non-invasive tests are inconclusive.
Abstinence: The most important treatment for ARLD at all stages. Abstinence can reverse fatty liver completely, improve alcoholic hepatitis, halt the progression of fibrosis, and improve the prognosis of cirrhosis. Even in advanced cirrhosis, abstinence improves survival and quality of life.
Nutritional support: Malnutrition is common in ARLD and worsens outcomes. Nutritional support — including adequate protein, calories, and micronutrients — is an important component of treatment.
Medications for alcoholic hepatitis: Corticosteroids (prednisolone) are used for severe alcoholic hepatitis to reduce inflammation. Pentoxifylline was previously used but has not been shown to improve survival in recent trials.
Liver transplantation: For people with end-stage liver disease who have achieved abstinence, liver transplantation can be life-saving. Most transplant programs require a period of abstinence (typically 6 months) before listing, though early transplantation for severe alcoholic hepatitis is increasingly practiced at some centers.

Can the liver recover from cirrhosis?
Cirrhosis is generally considered irreversible, but the liver can recover significant function with sustained abstinence. People with compensated cirrhosis who achieve abstinence have much better outcomes than those who continue drinking. Some people with early cirrhosis show regression of fibrosis with abstinence.
How much alcohol causes liver disease?
The risk of ARLD increases with the amount of alcohol consumed. Heavy drinking — more than 14 drinks per week for men or 7 for women — significantly increases risk. However, individual susceptibility varies — some people develop cirrhosis with relatively modest drinking, while others drink heavily for decades without developing cirrhosis.
Can I drink moderately if I have fatty liver?
No. Even moderate drinking can worsen fatty liver and increase the risk of progression to more severe disease. Complete abstinence is recommended for anyone with ARLD.
