Alcohol withdrawal syndrome (AWS) is a potentially life-threatening medical condition that occurs when a person with physical dependence on alcohol stops or significantly reduces their drinking. While many people experience only mild withdrawal symptoms, a significant minority develop severe complications including seizures and delirium tremens (DTs) — a condition with a mortality rate of 5-15% if untreated. Recognizing AWS, assessing its severity, and providing appropriate medical management are essential skills for anyone working in addiction medicine or supporting people in recovery from alcohol use disorder.
Alcohol withdrawal syndrome (AWS): The constellation of symptoms that occur when a person with physical dependence on alcohol stops or reduces drinking.
Delirium tremens (DTs): The most severe form of alcohol withdrawal, characterized by severe confusion, agitation, hallucinations, fever, and autonomic instability. Life-threatening if untreated.

Alcohol withdrawal is driven by the neurobiological adaptations that occur with chronic alcohol use. Alcohol enhances GABA (inhibitory) neurotransmission and inhibits glutamate (excitatory) neurotransmission. With chronic use, the brain adapts by reducing GABA receptor sensitivity and increasing glutamate receptor activity — in an attempt to maintain normal neural activity despite the presence of alcohol.
When alcohol is removed, these adaptations are unmasked: GABA function is reduced and glutamate function is increased, producing a state of neural hyperexcitability. This hyperexcitability produces the symptoms of AWS — anxiety, tremor, seizures, and in severe cases, delirium tremens.
AWS follows a characteristic timeline:
6-24 hours after last drink: Mild symptoms begin — anxiety, tremor, sweating, nausea, headache, insomnia, tachycardia, and hypertension. These symptoms reflect the initial neurobiological rebound.
24-48 hours: Symptoms intensify. Seizures can occur in this window — typically generalized tonic-clonic seizures that are usually brief and self-limiting. Approximately 5-10% of people with AWS experience seizures. Alcoholic hallucinosis — visual, auditory, or tactile hallucinations in a clear sensorium — can occur in this phase.
48-72 hours: Peak severity. Delirium tremens can develop in this window — characterized by severe confusion, agitation, hallucinations, fever, diaphoresis, and autonomic instability (tachycardia, hypertension). DTs occur in approximately 5% of people with AWS and carry a mortality rate of 5-15% if untreated.
Days 4-7: Gradual resolution of acute symptoms in most cases. Protracted withdrawal symptoms — anxiety, insomnia, cognitive impairment — can persist for weeks to months.
Not all people with AWS require inpatient medical management. Risk stratification is essential:

CIWA-Ar assessment: The CIWA-Ar scale assesses 10 domains: nausea/vomiting, tremor, paroxysmal sweats, anxiety, agitation, tactile disturbances, auditory disturbances, visual disturbances, headache, and orientation. Scores of 0-9 indicate mild withdrawal; 10-14 moderate; ≥15 severe.
Benzodiazepines: Benzodiazepines are the first-line treatment for AWS. They enhance GABA function, compensating for the reduced GABA activity of withdrawal, and prevent seizures and DTs. Long-acting benzodiazepines (diazepam, chlordiazepoxide) are preferred for their smooth, self-tapering effect. Lorazepam is preferred in patients with liver disease due to its simpler metabolism.
Two dosing strategies are used:
Thiamine (vitamin B1): Thiamine supplementation is essential in all patients with AWS to prevent Wernicke encephalopathy. Thiamine should be given before glucose administration, as glucose can precipitate Wernicke encephalopathy in thiamine-deficient patients. High-dose IV thiamine (500mg three times daily for 3 days) is recommended for patients at high risk of Wernicke encephalopathy.
Anticonvulsants: Carbamazepine, valproate, and gabapentin are used as alternatives or adjuncts to benzodiazepines in some settings. They are particularly useful in outpatient settings where benzodiazepine diversion is a concern.
Supportive care: IV fluids for hydration, electrolyte replacement (particularly magnesium and potassium), and nutritional support are important components of AWS management.
DTs is a medical emergency requiring intensive care unit management. Recognition is critical:
Clinical features: Severe confusion and disorientation, agitation, visual and tactile hallucinations (classically, seeing insects or small animals), fever, profuse sweating, tachycardia, and hypertension.
Management: High-dose benzodiazepines (IV diazepam or lorazepam), supportive care, and management of complications (hyperthermia, electrolyte abnormalities, aspiration pneumonia). Propofol or phenobarbital may be required for refractory DTs.
The kindling phenomenon — in which repeated withdrawal episodes become progressively more severe — has important clinical implications. People who have experienced multiple prior withdrawals are at higher risk for seizures and DTs with subsequent withdrawals, even if their current drinking level is lower than in previous episodes. This is one reason why early intervention and sustained treatment are important — preventing repeated withdrawal episodes reduces the risk of severe future withdrawals.
Can alcohol withdrawal be fatal?
Yes. Delirium tremens — the most severe form of alcohol withdrawal — carries a mortality rate of 5-15% if untreated. Even with treatment, DTs can be fatal. This is why medical supervision is essential for alcohol withdrawal in people with significant physical dependence.
Can I detox from alcohol at home?
Home detox may be appropriate for people with mild withdrawal symptoms and no high-risk features. However, it requires medical supervision, a reliable support person, and access to emergency care. People with a history of seizures or DTs, severe withdrawal symptoms, or significant medical or psychiatric comorbidities should detox in a medical setting.
How long does alcohol withdrawal last?
