Recovery from alcohol use disorder is not just psychological and neurological — it is profoundly physical. Years of heavy drinking deplete essential nutrients, damage organs, disrupt sleep, and impair the body's ability to repair itself. The good news is that the body is remarkably resilient. With the right support — including targeted nutrition and regular physical activity — physical recovery can be dramatic and relatively rapid.

This article focuses on the physical dimension of recovery: what alcohol does to the body, what nutrients are most commonly depleted, and how exercise and nutrition can accelerate healing and support long-term sobriety.

What Alcohol Does to the Body

Nutritional depletion. Alcohol is calorie-dense but nutritionally empty. Heavy drinkers often substitute alcohol calories for food calories, leading to significant nutritional deficiencies. Alcohol also impairs the absorption of nutrients from food and increases the excretion of water-soluble vitamins and minerals.

Liver damage. The liver is the primary site of alcohol metabolism. Chronic heavy drinking causes a progression of liver damage: fatty liver (reversible), alcoholic hepatitis (potentially reversible), and cirrhosis (largely irreversible). Even without cirrhosis, liver function is often significantly impaired in heavy drinkers.

Gastrointestinal damage. Alcohol damages the lining of the stomach and intestines, impairing nutrient absorption and causing inflammation. It disrupts the gut microbiome, which affects everything from immune function to mood.

Cardiovascular effects. While moderate alcohol consumption has been associated with some cardiovascular benefits (a relationship that is increasingly questioned), heavy drinking causes cardiomyopathy (weakening of the heart muscle), arrhythmias, and elevated blood pressure.

Brain and nervous system. Alcohol damages neurons and disrupts neurotransmitter systems. Thiamine (vitamin B1) deficiency, common in heavy drinkers, can cause Wernicke's encephalopathy and Korsakoff syndrome — serious neurological conditions.

Key Nutrients Depleted by Alcohol

Thiamine (Vitamin B1) is perhaps the most critical nutrient to address in early recovery. Thiamine deficiency can cause Wernicke's encephalopathy — a medical emergency characterized by confusion, eye movement abnormalities, and coordination problems. All people in alcohol recovery should receive thiamine supplementation, typically by injection in the acute phase and orally thereafter.

Folate (Vitamin B9) is commonly depleted in heavy drinkers. Folate deficiency contributes to anemia, cognitive impairment, and increased cancer risk. Good food sources include leafy greens, legumes, and fortified foods.

Vitamin B12 is often deficient due to impaired absorption. B12 is essential for neurological function and red blood cell production. Supplementation is often needed in early recovery.

Magnesium is depleted by alcohol and is involved in hundreds of enzymatic reactions. Magnesium deficiency contributes to anxiety, muscle cramps, insomnia, and cardiovascular problems. Foods rich in magnesium include nuts, seeds, leafy greens, and dark chocolate.

Zinc is important for immune function, wound healing, and taste and smell. Alcohol impairs zinc absorption and increases its excretion. Zinc deficiency is common in heavy drinkers.

Vitamin D deficiency is common in the general population and is exacerbated by heavy drinking. Vitamin D is essential for bone health, immune function, and mood regulation. Supplementation is often appropriate.

Omega-3 fatty acids support brain health and have anti-inflammatory effects. They are important for the neurological recovery that occurs in sobriety. Good sources include fatty fish, walnuts, and flaxseed.

Nutritional Strategies for Recovery

In early recovery, the goal is to replenish depleted nutrients and support the body's healing processes. This does not require a complicated or restrictive diet — it requires eating regularly, prioritizing nutrient-dense foods, and addressing specific deficiencies.

Eat regularly. Many people in early recovery have disrupted eating patterns. Establishing regular meals — three meals a day, with snacks as needed — stabilizes blood sugar, supports mood, and provides the nutrients the body needs for recovery.

Prioritize protein. Protein provides the amino acids needed for neurotransmitter synthesis (serotonin, dopamine, GABA) and tissue repair. Good sources include eggs, lean meats, fish, legumes, and dairy.

Eat plenty of vegetables and fruits. These provide vitamins, minerals, antioxidants, and fiber that support overall health and recovery. Leafy greens are particularly valuable for their folate, magnesium, and vitamin K content.

Stay hydrated. Alcohol is dehydrating, and the body needs adequate hydration for all its recovery processes. Aim for 8 or more glasses of water per day.

Limit sugar and processed foods. Many people in early recovery develop sugar cravings — the brain's reward system, depleted by the absence of alcohol, seeks other sources of dopamine stimulation. While some sugar is fine, excessive sugar consumption can destabilize mood and energy levels.

Exercise in Recovery

Exercise is one of the most powerful tools available for supporting recovery — and one of the most underutilized. The evidence for exercise in addiction recovery is substantial: it reduces cravings, improves mood, reduces anxiety and depression, improves sleep, supports neurological recovery, and provides a healthy source of dopamine stimulation.

Aerobic exercise (running, cycling, swimming, brisk walking) has the strongest evidence for mood improvement and craving reduction. It increases brain-derived neurotrophic factor (BDNF), a protein that supports neuronal growth and repair — essentially fertilizer for the recovering brain.

Strength training improves body composition, increases energy, and provides a sense of accomplishment and agency. Many people in recovery find that building physical strength is a powerful metaphor for and complement to building emotional and psychological strength.

Yoga combines physical movement with mindfulness and breath awareness — making it particularly valuable for recovery. It reduces stress, improves body awareness, and supports the emotional regulation skills that are central to sustained sobriety.

The key is to start where you are. If you have not exercised in years, a 20-minute walk is a meaningful beginning. The goal is consistency, not intensity. Building a regular exercise habit — even a modest one — provides benefits that compound over time and become one of the most reliable pillars of a sober life.

The Body as an Ally

Many people in active addiction have a complicated, adversarial relationship with their bodies — using substances to numb physical sensations, ignoring physical needs, and experiencing the body primarily as a source of craving and discomfort. Recovery offers the opportunity to develop a different relationship: one of care, attention, and partnership.

Attending to the body's needs — through nutrition, exercise, sleep, and medical care — is not just a strategy for physical health. It is a practice of self-respect, a daily affirmation that you are worth caring for. That practice, repeated over time, becomes one of the foundations of a sober life.
