Sleep disorders and addiction are deeply intertwined. Substance use disrupts sleep architecture, and sleep problems are among the most common and most persistent symptoms of early recovery. Poor sleep is a significant relapse trigger — sleep deprivation increases craving, impairs decision-making, and reduces stress resilience. Understanding the relationship between sleep and addiction — and developing evidence-based approaches to improving sleep in recovery — is essential for sustainable recovery.
Sleep architecture: The structure of sleep, including the proportion of time spent in different sleep stages (NREM stages 1-3 and REM sleep). Disrupted by substance use.
REM sleep: Rapid eye movement sleep — the stage of sleep associated with dreaming, emotional processing, and memory consolidation. Suppressed by alcohol and many other substances.
Insomnia: Difficulty falling asleep, staying asleep, or waking too early, with associated daytime impairment. Extremely common in early recovery.
Post-acute withdrawal syndrome (PAWS): A prolonged withdrawal syndrome that can last weeks to months after stopping substances. Insomnia is a common PAWS symptom.
CBT-I (Cognitive Behavioral Therapy for Insomnia): The gold-standard treatment for chronic insomnia. More effective than sleep medications in the long term.
Different substances disrupt sleep through different mechanisms:
Alcohol: Alcohol is commonly used as a sleep aid — it reduces sleep onset latency (time to fall asleep). However, it significantly disrupts sleep architecture — suppressing REM sleep, causing early morning awakening, and producing fragmented sleep in the second half of the night. Chronic alcohol use leads to tolerance to its sleep-inducing effects, requiring increasing amounts to achieve the same effect.
Opioids: Opioids suppress REM sleep and slow-wave sleep, reducing sleep quality. They also cause central sleep apnea — a potentially dangerous breathing disorder during sleep. Opioid withdrawal produces severe insomnia.
Stimulants: Stimulants (cocaine, methamphetamine, amphetamines) suppress sleep and reduce total sleep time. Stimulant withdrawal produces hypersomnia (excessive sleepiness) followed by rebound insomnia.
Cannabis: Cannabis reduces REM sleep and may improve sleep onset in the short term. However, chronic cannabis use is associated with reduced sleep quality, and cannabis withdrawal produces insomnia and vivid dreams.
Benzodiazepines: Benzodiazepines reduce sleep onset latency and increase total sleep time in the short term. However, they suppress slow-wave sleep and REM sleep, reducing sleep quality. Benzodiazepine withdrawal produces severe rebound insomnia.
Sleep problems are among the most common and most persistent symptoms of early recovery:
Insomnia: Insomnia is extremely common in early recovery — affecting 50-80% of people in the first weeks of abstinence. It typically improves over weeks to months but can persist for longer.
REM rebound: After stopping substances that suppress REM sleep (alcohol, opioids, cannabis), there is a rebound increase in REM sleep — producing vivid, disturbing dreams that can disrupt sleep and trigger craving.
Hypersomnia: After stopping stimulants, people often experience hypersomnia — excessive sleepiness — as the brain recovers from stimulant-induced sleep deprivation.
Poor sleep is a significant relapse trigger:
Increased craving: Sleep deprivation increases craving for substances — particularly stimulants and alcohol. Research has found that even one night of poor sleep significantly increases craving in people in recovery.
Impaired decision-making: Sleep deprivation impairs prefrontal cortex function, reducing the capacity for rational decision-making and increasing impulsivity. Under sleep deprivation, people are more likely to make impulsive decisions — including the decision to use substances.
Reduced stress resilience: Sleep deprivation increases HPA axis reactivity and reduces stress resilience, increasing vulnerability to stress-induced relapse.
CBT-I (Cognitive Behavioral Therapy for Insomnia): CBT-I is the gold-standard treatment for chronic insomnia and is more effective than sleep medications in the long term. It includes sleep restriction therapy, stimulus control, sleep hygiene education, and cognitive restructuring of unhelpful beliefs about sleep. CBT-I has been shown to be effective for insomnia in people with addiction.
Sleep hygiene: Basic sleep hygiene practices — consistent sleep and wake times, a dark and cool sleep environment, avoiding screens before bed, avoiding caffeine after noon — can significantly improve sleep quality.
Exercise: Regular aerobic exercise significantly improves sleep quality and reduces insomnia. Exercise should be completed at least 3 hours before bedtime to avoid stimulating effects.
Mindfulness: Mindfulness meditation reduces the hyperarousal and racing thoughts that contribute to insomnia. MBSR has been shown to improve sleep quality in people with insomnia.
Medications: Sleep medications should be used with caution in people with addiction. Non-addictive options include melatonin, trazodone, and low-dose doxepin. Benzodiazepines and Z-drugs (zolpidem, eszopiclone) should generally be avoided due to their addiction potential.
How long does insomnia last in recovery?
Insomnia typically improves significantly over the first few weeks to months of recovery. However, for some people — particularly those with alcohol use disorder or benzodiazepine dependence — insomnia can persist for longer. CBT-I is effective for persistent insomnia in recovery.
Can I take melatonin in recovery?
Yes. Melatonin is a non-addictive sleep aid that can be helpful for sleep onset difficulties in early recovery. It is most effective for circadian rhythm disruptions (jet lag, shift work) rather than maintenance insomnia.
