But alcohol is a systemic toxin that affects virtually every organ in the body. Understanding the physical consequences of heavy drinking — and the remarkable capacity of the body to heal in recovery — is important for anyone navigating alcohol use disorder.
The liver is the organ most associated with alcohol damage, and for good reason: it is the primary site of alcohol metabolism, and it bears the brunt of chronic heavy drinking. Alcohol-related liver disease progresses through three stages: fatty liver (steatosis), alcoholic hepatitis, and cirrhosis.
Fatty liver occurs in virtually everyone who drinks heavily — the liver accumulates fat as a byproduct of alcohol metabolism. It is reversible with abstinence; the liver typically returns to normal within weeks of stopping drinking.
Alcoholic hepatitis is inflammation of the liver caused by alcohol. It ranges from mild (often asymptomatic) to severe (life-threatening). Severe alcoholic hepatitis has a mortality rate of up to 50% within 30 days. Abstinence is the most important treatment.
Cirrhosis is irreversible scarring of the liver that develops after years of heavy drinking. It impairs liver function, causes portal hypertension, and can lead to liver failure. Cirrhosis is not reversible, but abstinence can slow its progression and significantly improve survival.
Heavy drinking is a leading cause of cardiomyopathy (weakening of the heart muscle), arrhythmias (irregular heart rhythms), and hypertension (high blood pressure). "Holiday heart syndrome" — the occurrence of arrhythmias after binge drinking — is well-documented. Long-term heavy drinking increases the risk of heart failure, stroke, and sudden cardiac death.
The relationship between alcohol and heart disease is complex: moderate drinking (1-2 drinks per day) has been associated with reduced cardiovascular risk in some studies, but this association is controversial and does not justify drinking for cardiovascular benefit. Heavy drinking unambiguously increases cardiovascular risk.
Chronic heavy drinking causes direct neurotoxicity — killing neurons and reducing brain volume. The prefrontal cortex, cerebellum, and hippocampus are particularly vulnerable. The consequences include: cognitive impairment (memory, attention, executive function), cerebellar ataxia (impaired coordination and balance), and peripheral neuropathy (numbness, tingling, and pain in the hands and feet).
Wernicke-Korsakoff syndrome — a severe neurological condition caused by thiamine (vitamin B1) deficiency associated with heavy drinking — produces acute confusion, eye movement abnormalities, and ataxia (Wernicke's encephalopathy), followed by severe memory impairment (Korsakoff's psychosis). It is preventable with thiamine supplementation and treatable if caught early.
Heavy drinking is a leading cause of pancreatitis — inflammation of the pancreas that can be acutely life-threatening and chronically debilitating. It also increases the risk of gastritis, esophageal varices (enlarged veins in the esophagus that can rupture and bleed), and gastrointestinal cancers (esophageal, stomach, colorectal).
Alcohol is a Group 1 carcinogen — a substance with established evidence of causing cancer in humans. Heavy drinking increases the risk of cancers of the mouth, throat, esophagus, liver, colon, rectum, and breast. The risk increases with the amount consumed and is compounded by smoking.
The remarkable thing about the human body is its capacity for recovery. Many of the physical effects of heavy drinking are reversible with sustained abstinence. Fatty liver resolves within weeks. Blood pressure improves within months. Brain volume increases with sustained sobriety — studies show measurable increases in gray matter volume after 6-12 months of abstinence. Peripheral neuropathy often improves significantly. Even liver function can improve substantially with abstinence, even in people with significant liver disease.
The body wants to heal. Giving it the opportunity — through sustained abstinence, good nutrition, exercise, and medical care — is one of the most important gifts of recovery.
